Сегодня в результате отключки компьютерной системы использующейся для управления полетов все полеты в стране были отменены : 

 

… flights were grounded this morning due to an outage with an important Federal Aviation Administration (FAA) system for pilots.

 

ABC News reported that a senior official briefed on the matter said the software issue led to a “cascading” series of IT failures, leading to this morning’s flight groundings.

As of 7:15 a.m., more than 1,200 flights had been delayed into or out of the U.S., which was shortly before the FAA announced the pause.

причина этой отключки найдена :

… ground stop order that paused all domestic departures and the FAA systems failures on Wednesday appeared to have been the result of a mistake that occurred during routine schedule systems maintenance. 
… the engineer "replaced one file with another," not realizing a mistake was being made. 

получается что в этой важной системе управления полетами нет «защиты от дураков» и малейшее случайное нажатие на кнопочку может ее обрушить.
